Our Service
If you have been admitted into Northwick Park hospital and are now ready to be discharged, you may be offered our FREE discharge Support Service where one of our support workers will escort you home by taxi.
Our team will ensure you get home safely, help you get settled, can carry out some small household tasks if needed and if necessary offer you a daily call for up to three days following discharge.
How we do it
Leaving the hospital
- Your discharge will be organised by your care team at the hospital and they will give you letters, medication, & equipment as needed, to help so that you can stay well. They will also let your GP know that you are home.
- Your recovery is important and our priority is that you are discharged safely and successfully from the hospital.
Our dedicated team is available to support and assist with:
- Transport arrangements (they may be able to escort you home)
- Get you settled in your home (ensure you have a change of clothes and personal care items, purchasing food items, adequate heating in your home, etc)
- Picking up and delivering medication
- A daily call after your discharge
- Carry out a full assessment of needs and organise help and support for you from the relevant internal and external services

Age UK Hillingdon Harrow Brent and Harrow Carers delivering support through the Harrow Together Partnership. We support older residents 60+ and Unpaid Carers.

Regular Outings
We organise regular trips that take carers out to relax, learn and enjoy themselves in a range of different places.

- Hasting’s Pier
- Headstone Manor Museum
